If you work in the NICU-to-Early Intervention-to-Private Practice pipeline, then you are already intimately aware of struggle that caregivers encounter with ensuring continuity of care between settings, as well as the mental health struggles that they can encounter along the way and you too probably wonder, “how can we better serve these little ones and their families?” Tiffany turned her worries into victories for caregivers by establishing a clear path from NICU discharge into community care teams and establishing a support system to ensure that the mental health of the infant-caregiver dyad is thriving!
