Beyond the Needle: Botox’s Hidden Impact on Speech & Swallowing

Beyond the Needle: Botox’s Hidden Impact on Speech & Swallowing

Botulinum toxin is widely used to correct fine lines and wrinkles, but what happens when its effects extend beyond the intended target? In this episode of Brainstorms: Functional Rehab for SLPs, Renee Garrett, MSEd, CCC-SLP, CBIS is joined by Megan McCue, MS, CCC-SLP to take a closer look at the mechanism of action behind Botox and how localized chemodenervation can, in some cases, lead to broader, systemic consequences.

Together, they explore the overlap between botulinum toxin adverse effects and the domains SLPs treat every day, including dysphagia, dysarthria, and changes that may be misattributed to cognitive decline or fatigue. When these symptoms emerge, they can blur the clinical picture and raise important questions about etiology, timing, and patient safety. The conversation also addresses a critical gap in practice: why toxin-related adverse effects, including iatrogenic botulism, are often underrecognized or dismissed, and how SLPs can sharpen clinical reasoning, identify red flags, and advocate effectively when something doesn’t align with the expected course of recovery.

You’ll also walk away with practical next steps when botulinum toxin involvement is suspected, from targeted assessment and documentation to communication with prescribing providers and care teams. This episode helps clinicians move beyond surface-level understanding and respond with greater precision when Botox’s effects don’t follow the expected course.
